Transitioning Patients from Acute to Long Term Care

In order to provide a smooth transition out of the acute care environment, caregivers and discharge planners must be aware that different types of service levels are available in the long term care environment. The need for long term care services, matching patient needs with facility capabilities, critical elements of transfer orders, and quality indicators and reporting will be discussed.
